To enrol your child/children at Carbrook State School, you will need to complete and return the following forms. These forms can also be found in the enrolment pack available at the school office.
a completed enrolment form (PDF, 2.2MB)
a completed enrolment agreement (PDF, 150KB)
a completed internet access for student use agreement (PDF, 22KB)
a completed use of student image in media form (PDF, 261KB)
a completed religious education form (PDF, 59KB)
provide student's birth certificate or extract
visa and passport documentation if the student is not an Australian citizen
any other relevant documents, for example, previous school reports, medical documents, speech therapy reports, some samples of recent school work, would also be valuable for the future teacher.
As part of the Education Queensland school system, Carbrook State School has an enrolment managed area that you can access on the Department of Education's school catchment area EdMap website.

Term 4 is the time when we collaborate with
early childhood and care services (ECECs) and engage with new families and
their children to prepare for a smooth transition to Prep and ensure they have
a great start to learning.
Kindergarten transition statements are a key resource
for this collaboration, providing schools with the opportunity to get to know
the child and their family and plan for the transition to school.
To facilitate the sharing of transition statements
between early childhood education and care (ECEC) services and schools, the
Department has created a generic transition email address for schools to
receive transition statements for new enrolments.
